Credit Card Slip
|
- Unfortunately due to a rash of stolen credit cards
being used from outside the United States and Canada to place orders
over $100 or Custom Crafted items at the Celtic Attic, we are requiring that you send us a copy of your
credit card and your signature authorizing the purchase.
- Click on the graphic of the Credit Card below.
Right Click and SAVE PICTURE AS to your computer. You can
then print the Credit Card slip. Please place your credit
card under the paper and rub with a pencil to get an imprint of the
card. Then sign the bottom of the slip. Please cross out
all but the Last 4 Digits of the credit card number. This will
keep the Security of your Credit Card data in tact.
- Mail it to the
Celtic Attic - 150 Elk Road Quilcene WA 98376.
- A Credit Card slip will be emailed out with each
order received from outside the USA and for all orders over $100.00.
